Accountability and human oversight

Mental model: oversight is a control loop around a socio-technical system, not a person placed at the end of a model. Accountability answers who has authority to decide, what evidence they need, what they can change, and how the decision remains auditable. A human who can only click “approve” has neither meaningful control nor the responsibility that organizations often try to assign to them.

Assign decision rights by lifecycle stage

Use a named role for each cell—not “the AI team.” One person may hold several roles in a small organization, but the decision and escalation path must remain explicit.

This is intentionally simpler than a RACI sheet. The practical test is whether an incident commander can locate the decision record, pause button, and responsible role in minutes.

Make human review effective

For a consequential decision, show the reviewer the purpose, affected person or object, source provenance, uncertainty, applicable policy, alternatives, and the reversible action they are authorizing. Record the version of model, prompt, data or retrieval corpus, and policy that produced the proposal. Give the reviewer time and a way to reject with a reason; feed that reason into the appeal and evaluation workflow.

Measure false approvals, false rejections, workload, decision latency, override rate, and successful appeals by group. A low override rate is ambiguous: it can signal good automation or powerless reviewers. Audit samples and user recourse resolve that ambiguity.

Failure modes and decision rule

  • Accountability theater: a committee “owns” risk but cannot halt deployment.
  • Rubber-stamping: evidence is too opaque or volume is too high to review.
  • Automation bias: reviewers defer to a score without calibrated uncertainty.
  • No recourse: affected people cannot correct data or challenge a decision.

Require pre-action review for high-impact or irreversible actions; use on-the-loop monitoring only where delay would create greater harm and a rapid stop mechanism exists. This is governance guidance, not legal advice—map the deployed jurisdiction and sector with qualified counsel.
